1. Agreement
These Terms of Use (the “Terms”) are an agreement between you and LA Integrative Nursing Inc., doing business as LA Integrative Psychiatry (“LAIP,” “we,” “us,” or “our”), for your use of laip.us (the “Site”). By using the Site you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Site. We may change these Terms at any time by posting a new version with a new date; your continued use of the Site after a change means you accept it.
2. The Site is for information and contact, not medical advice
Content on the Site is general health information. It is not medical advice, does not create a provider-patient relationship, and is not a substitute for an evaluation by a licensed clinician who knows your history. Do not start, stop, or change a medication or treatment based on something you read here. Treatments described on the Site may help some people; results vary from person to person.
3. Emergencies
If you are in crisis, think you may hurt yourself or someone else, or have a medical emergency, call 911 or 988 (Suicide and Crisis Lifeline), or go to the nearest emergency room. Do not use the Site, its forms, email, or online booking for emergencies. They are not monitored around the clock.
4. Becoming a patient
Submitting a form or an online booking request is a request for an appointment. It does not make you a patient. A provider-patient relationship begins only when we accept you as a patient and you complete our intake, consent, and financial forms. We may decline a request for any lawful reason. Ongoing psychiatric care at our practice is for adults 18 and older who are located in California. ADHD testing for children and teens is arranged through a parent or legal guardian, who must attend the visit and sign consent.
5. Telehealth
We offer video visits only to patients who are physically located in California at the time of the visit, through a separate secure platform, and only after you sign a telehealth consent. The Site itself does not provide telehealth and does not transmit clinical information.
6. Patient portal and third-party accounts
Online booking and the patient portal are provided through Tebra and are subject to Tebra’s terms in addition to these Terms. Keep your login private. You are responsible for activity under your login and must tell us promptly if you think it has been used without permission.
7. Calls, texts, and email
7.1 Care-related messages
By giving us a phone number or email address, you agree that we may contact you at that number or address about your request, your appointments, billing, and your care, including by text message. These messages are not marketing. Message and data rates may apply.
7.2 Marketing messages (optional)
If you check the marketing box on one of our forms, or otherwise give us your written consent, you agree that LA Integrative Psychiatry may send recurring marketing text messages and emails about our own services, office news, and general health information to the mobile number and email address you provided, including messages sent with automated technology. Consent is not a condition of receiving care or of buying any service. Message frequency varies, up to 4 messages per month. Message and data rates may apply. Reply HELP for help. Reply STOP to cancel at any time; we will send one message confirming that you are unsubscribed. You may also opt out by calling (747) 777-7047, emailing [email protected], or telling us in any other reasonable way, and we will honor your request within 10 business days. To opt out of marketing email, use the unsubscribe link in the email. Mobile carriers are not liable for delayed or undelivered messages. We send marketing texts only between 8 a.m. and 9 p.m. in your local time.
7.3 Privacy of messages
Our messages never include your diagnosis, condition, medication, or appointment details. Text messages and ordinary email are not secure channels; please do not send us detailed medical information, Social Security numbers, or payment card numbers by text or email. Use the patient portal or call our office instead. We do not share your mobile number or text opt-in data with third parties for their marketing.
7.4 Your number
You confirm that you are the subscriber or regular user of the phone number you give us. If your number changes or you give up the number, please tell us so messages do not reach someone else.
8. Accuracy of the Site
We work to keep the Site accurate and current, and clinical content is reviewed by a licensed provider. We make no promise that the Site is free of errors or that it describes every service, price, or insurance plan correctly at every moment. Prices, hours, and insurance participation can change; call us to confirm.
9. Acceptable use
You agree not to: use the Site to break the law; submit false information or impersonate another person; collect information about other users; scrape, crawl, or copy the Site except as search engines do in the ordinary course; interfere with the Site’s security or operation; upload malware; or use the Site to harass, threaten, or harm anyone. We may limit or block access to the Site for any of these reasons.
10. Intellectual property
The Site, its text, images, design, logo, and the names “LA Integrative Psychiatry” and “LAIP” are owned by LAIP or licensed to us and are protected by copyright and trademark law. You may view and print pages for personal, non-commercial use. Any other use, including copying content for a commercial purpose, requires our written permission. Third-party names and marks that appear on the Site, such as QbTest, QbCheck, Tebra, and Google, belong to their owners.
11. Third-party links and embedded tools
The Site links to and embeds services operated by others, including the Tebra scheduling tool, Google Maps, a Google reviews widget, and provider directories. We do not control those services and are not responsible for their content, availability, or privacy practices. Links do not mean we endorse a service.
12. Reviews and public comments
If you choose to post a public review about us on any platform, please do not include another person’s health information. Because of federal and California privacy law, we cannot confirm or deny in public whether any person is or was our patient, and our public replies are limited to general statements.
13. Disclaimer of warranties
THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE.” TO THE FULLEST EXTENT PERMITTED BY LAW, LAIP DISCLAIMS ALL WARRANTIES ABOUT THE SITE, WHETHER EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. THIS SECTION APPLIES TO THE SITE ONLY. IT DOES NOT LIMIT ANY DUTY WE OWE YOU AS A PATIENT UNDER YOUR TREATMENT AGREEMENTS OR APPLICABLE LAW.
14. Limitation of liability
TO THE FULLEST EXTENT PERMITTED BY CALIFORNIA LAW, LAIP AND ITS OWNERS, PROVIDERS, EMPLOYEES, AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF, OR INABILITY TO USE, THE SITE. THIS SECTION LIMITS LIABILITY FOR THE SITE ONLY. IT DOES NOT LIMIT OR WAIVE ANY LIABILITY FOR PROFESSIONAL SERVICES PROVIDED TO YOU AS A PATIENT, WHICH IS GOVERNED BY YOUR TREATMENT AGREEMENTS AND APPLICABLE LAW, AND IT DOES NOT LIMIT LIABILITY THAT CANNOT BE LIMITED UNDER CALIFORNIA LAW.
15. Governing law and venue
These Terms are governed by the laws of the State of California without regard to its conflict-of-laws rules. Any dispute about the Site or these Terms will be brought in the state or federal courts located in Los Angeles County, California, and you consent to the jurisdiction of those courts.
16. Accessibility
We want everyone to be able to use the Site and aim to meet WCAG 2.1 Level AA guidelines. If you have trouble using any part of the Site, call (747) 777-7047 or email [email protected] and we will provide the information in another format or help you schedule by phone.
17. General
If any part of these Terms is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver of it. These Terms, together with the Privacy Policy, are the entire agreement between you and LAIP about the Site. They do not replace the intake, consent, and financial agreements you sign as a patient.
