These are the policies every LAIP patient agrees to. You will also sign them before your first visit as part of your intake paperwork. If anything here is unclear, call us at (747) 777-7047 before you book.

Appointments and cancellations

We ask for at least 48 hours notice to cancel or reschedule. You can call or text (747) 777-7047, use the patient portal, or use the cancel link in your appointment reminder.

  • Cancellations with less than 48 hours notice, and missed appointments, are charged a $100 fee.
  • The fee applies to in-person and telehealth visits and is charged to the card on file.
  • Insurance does not cover this fee. Medi-Cal patients are not charged it.
  • For telehealth, if we cannot reach you within 10 minutes of your start time, the visit counts as a no-show.
  • After three missed appointments in 12 months, we may ask you to prepay for future visits.
  • Emergencies happen. If something serious kept you from your visit, tell us and we will review it.

Insurance and payment

We are in network with Aetna, Anthem Blue Cross, Blue Shield of California, Cigna and Evernorth, Magellan, Medicare, Optum, Oxford, and UnitedHealthcare. Some plans are covered through our Headway network participation. As a courtesy we bill your insurance for you. Your plan, not LAIP, decides what it covers, and you are responsible for anything it does not pay. This includes copays, coinsurance, deductibles, denied services, and services that needed a prior authorization your plan did not approve.

Copays and self-pay fees are due at check-in, before you see your provider. After your plan processes a claim, any remaining balance is yours to pay and is due within 30 days of the statement date.

Self-pay rates and your Good Faith Estimate

If you do not have insurance, or you choose not to use it, our self-pay rates are $400 for the initial evaluation and $200 for a follow-up visit.

Under the federal No Surprises Act, self-pay and uninsured patients have the right to a written Good Faith Estimate of expected charges before scheduled care. Ask for one at any time by calling (747) 777-7047 and we will provide it within 3 business days. The estimate is not a bill, and your actual charges may differ if your care changes. If you receive a bill at least $400 more than your estimate for that service, you may dispute it through the federal patient-provider dispute resolution process within 120 days of the bill date. Read our full Good Faith Estimate notice, and find federal details at www.cms.gov/nosurprises.

Credit card on file

We require every patient to keep a valid credit, debit, or HSA/FSA card on file as a condition of scheduling non-emergency care. Your card is stored by our PCI-compliant payment processor, not on paper in our office. We never ask for or store your card security code.

You sign a separate authorization that lists exactly what we can charge: copays and visit fees, balances remaining after insurance, missed appointment fees, and fees for forms and records.

  • Visit fees and copays are charged on the day of the visit.
  • Balances after insurance are charged after we receive the explanation of benefits and send you a statement.
  • For any single charge above $500, we notify you by text, email, or phone at least 5 business days beforehand so you can ask questions first.
  • You receive an itemized receipt for every charge.
  • If your card expires or is declined, please give us a new one within 7 days.
  • You may cancel the authorization in writing at any time. Cancelling does not remove amounts you already owe.
  • Medicare and Medi-Cal patients are charged only what those programs allow.

Medication and controlled substances

Controlled substances, including stimulants for ADHD, are not prescribed at your first visit. Before any controlled substance is considered, your provider completes a comprehensive psychiatric evaluation, reviews your medication history, orders a drug screen, and checks California’s prescription monitoring database (CURES). This usually takes more than one appointment, so please plan for that. Whether a medication is prescribed afterward is a clinical decision made by your provider.

If you are already taking a controlled substance from another provider, bring your prescription bottles and that prescriber’s contact information to your first visit so your care can continue safely. Controlled medications are not refilled early and are not replaced if lost or stolen. Refill requests between visits need 3 business days.

Telehealth

We offer telehealth to patients located anywhere in California. You will confirm your identity and your current location at the start of each visit, and you should be in a private space, not driving. Telehealth visits are billed the same as in-person visits. Telehealth is not for emergencies. If you are in crisis, call 911 or the 988 Suicide and Crisis Lifeline, or go to the nearest emergency room.

Forms, letters, and records

  • Copies of your medical records: fee set by California Health and Safety Code section 123110. Please allow up to 15 days.
  • Letters and forms completed outside a visit, such as work, school, or disability forms: $50 per form. These are provided only after your provider has completed a full assessment, and in most cases after 3 to 6 months of treatment.
  • No official documentation is signed or issued at a first appointment.

Billing questions

After each visit you receive a receipt listing the date, your provider, and the services billed. Please review it. If something looks wrong, contact us within 30 days at (747) 777-7047 or through the patient portal, and we will look into it and respond within 10 business days. Charges appear on your statement as LA Integrative Psychiatry or its payment processor. Please contact us before contacting your bank if you do not recognize a charge. Most billing questions are resolved quickly this way.

Unpaid balances and refunds

If a balance is not paid within 60 days of the statement date and no payment plan has been arranged, the account may be referred to a collection agency. Payment plans are available, so please ask before a balance becomes overdue. If we need to end care for nonpayment, we give 30 days written notice and information on finding other care. If you overpay, or your insurance later pays more than expected, we refund the difference to your original payment method within 30 days of confirming the overpayment.

Communication

We may contact you about scheduling and billing by phone, text message, email, or the patient portal using the contact information you give us. Message and data rates may apply, and you can opt out of text messages at any time by replying STOP. Marketing messages are sent only if you opt in; see our Terms of Use. For non-urgent clinical questions, use the portal or call during business hours and we will respond within 2 business days.
