LA Integrative Psychiatry is the business name of LA Integrative Nursing Inc., a California corporation (“LAIP,” “we,” “us,” or “our”). This Privacy Policy explains what information we collect when you visit laip.us (the “Site”), how we use it, who we share it with, and the choices you have.

This policy covers the Site. Your medical information is protected by federal law (HIPAA) and California law (the Confidentiality of Medical Information Act). How we use and protect medical information is described in our Notice of Privacy Practices. If this policy and the Notice of Privacy Practices ever say different things about medical information, the Notice of Privacy Practices controls.

1. Information we collect

Information you give us

  • Contact forms. When you use a form on the Site, such as a call-back request, an ADHD testing request, or a facilities partnership inquiry, we collect what you type: for example your name, phone number, email address, preferred contact time, insurance plan, and anything you write in the message box.
  • Online booking. When you request an appointment through the scheduling tool on our Book page, the information you enter goes to our scheduling and records system (Tebra). That information is part of your health record and is handled under our Notice of Privacy Practices.
  • Calls and texts. When you call or text our office, we receive your phone number and the content of the call or message. Our phone and messaging service is provided by Spruce Health.
  • Email. When you email us, we receive your email address and the content of your message.

Please use website forms for contact requests only. Do not include detailed medical history, Social Security numbers, or payment card numbers in a form or an email. We collect clinical details securely during intake and your visit.

Information collected automatically

When you visit the Site, we and our service providers collect technical information through cookies and similar tools (see Section 4): your IP address (shortened before it is stored for analytics), browser and device type, operating system, the pages you view, how long you stay, the website that referred you, approximate location at the city level, and clicks on buttons such as “Book” or “Call.”

Information from other sources

If you arrive at the Site from one of our ads, Google sends us a click identifier so we can measure whether the ad led to a booking click or a call. We display public Google reviews on the Site through a widget; we do not receive your private Google account information.

2. How we use information

  • To respond to your request and to schedule, confirm, and remind you of appointments
  • To verify insurance benefits when you ask us to
  • To communicate with you about your request, appointments, billing, and care by phone, email, or text
  • With your separate opt-in consent only, to send occasional marketing messages by text or email about our own services, office news, and general health information (see Section 5)
  • To run, secure, and improve the Site, and to understand which pages and ads bring people to us
  • To comply with the law, respond to lawful requests, and protect the rights, safety, and property of patients, staff, and others

We do not sell personal information. We do not share personal information with third parties for their own advertising. We do not build advertising audiences from visits to this Site, and we do not use remarketing. We never use information from substance use disorder treatment records for marketing, and we never use a diagnosis, condition, or medication to decide who receives a marketing message.

3. Who we share information with

  • Service providers that work for us under contract: web hosting, security and content delivery, analytics (Google Analytics), advertising measurement (Google Ads), scheduling and records (Tebra), phone and messaging (Spruce Health), email delivery, data backup, and billing partners. Service providers that handle protected health information sign HIPAA business associate agreements with us and may use the information only to perform services for us.
  • Health plans and other covered entities, as described in our Notice of Privacy Practices.
  • Legal and safety: when required by law, subpoena, or court order, or when we believe disclosure is necessary to protect someone’s safety.
  • Business transfers: if the practice is sold, merged, or reorganized, information may transfer to the successor under the same protections described here and in the Notice of Privacy Practices.

Mobile information. We do not sell, rent, or share mobile phone numbers or text-messaging opt-in data and consent with third parties or affiliates for their marketing or promotional purposes. Our text-messaging provider receives your number only to deliver our messages.

4. Cookies and similar technologies

Cookies are small files stored on your device. We use these types:

  • Strictly necessary (LAIP, Cloudflare, WordPress): security, bot filtering, load balancing, and basic site functions. Kept for a session up to 30 minutes; staff login cookies apply only to our staff.
  • Analytics (Google Analytics, cookies named _ga and _ga_*): counts visits and page views so we can see which pages are useful. Data is aggregated and IP addresses are shortened. Cookies last up to 2 years on your device; Google keeps event data for 2 months.
  • Advertising measurement (Google Ads, cookies named _gcl_au, _gcl_aw and similar): records that a click on our ad led to a booking click or a call. We do not use these cookies to build audiences or to show you ads on other sites. Up to 90 days.
  • Embedded content (Tebra on the Book page, Trustindex and Google in the reviews widget, Google for the map and fonts): makes the embedded tool work and may let that provider recognize your browser. See each provider’s privacy policy.

Where tags run. Analytics and advertising measurement tags run on the public pages of the Site only. They do not run inside the scheduling tool, the patient portal, or video visits.

Your choices. You can block or delete cookies in your browser settings; the Site still works, though a few features may not. You can also install the Google Analytics opt-out browser add-on (tools.google.com/dlpage/gaoptout) and adjust Google ad settings at adssettings.google.com.

Do Not Track and Global Privacy Control. The Site does not change its behavior in response to “Do Not Track” browser signals because there is no accepted standard for them. We treat a Global Privacy Control signal as a request not to sell or share your personal information. We do not sell or share personal information in any case.

5. Your rights and choices

  • Access and correction. You may ask us to review, correct, or delete information you gave us through the Site, subject to the laws that require us to keep medical and billing records.
  • Care-related messages. We contact you about your request, appointments, billing, and care at the phone number and email you give us. These messages do not require a marketing opt-in.
  • Marketing messages are optional. We send marketing texts or emails only if you check the marketing box on a form or give us other written consent. Consent is never a condition of receiving care. Text messages: message frequency varies, up to 4 per month; message and data rates may apply; reply HELP for help or STOP to opt out. Emails: use the unsubscribe link in any marketing email. You may also opt out by calling (747) 777-7047, emailing [email protected], or telling us in any other reasonable way. We honor opt-outs within 10 business days, and opting out never affects your care.
  • What our messages never include. Marketing messages never mention your diagnosis, condition, medication, or appointment details, because messages can appear on a locked phone screen.
  • Medical records. Your rights to access, amend, and receive an accounting of disclosures of your medical record are described in our Notice of Privacy Practices.
  • California residents. See Section 9.

6. How we protect information

We use reasonable administrative, technical, and physical safeguards: encrypted connections (HTTPS) across the Site, security monitoring and bot filtering, access limited to staff who need it, written agreements with service providers, and regular backups. No method of transmission or storage is completely secure. Website forms and ordinary email are not encrypted end to end, so please do not send sensitive details through them; use the patient portal or call our office instead. If a breach of your information occurs, we will notify you as the law requires.

7. How long we keep information

  • Form submissions are emailed to our staff; copies may be kept on our web server for a short time for backup and spam control. If you become a patient, your contact details become part of your record.
  • Analytics event data is kept by Google Analytics for 2 months. Aggregated reports do not identify you.
  • Server and security logs are typically kept for 30 days.
  • Medical and billing records are kept for the period required by California and federal law and by our records retention policy.

8. Children

The Site is intended for adults 18 and older. Ongoing psychiatric care at our practice is for adults 18 and older. ADHD testing for children and teens is arranged by a parent or legal guardian, who provides the information and signs consent in our office. We do not knowingly collect personal information online from children under 13. If you believe a child has sent us information through the Site, contact us and we will delete it.

9. California privacy rights

California law gives residents certain rights over personal information held by businesses. Medical information we hold as a health care provider is governed by HIPAA and the Confidentiality of Medical Information Act, and is exempt from the California Consumer Privacy Act. For other personal information collected through the Site, you may ask us what personal information we have collected about you, ask us to delete it, or ask us to correct it. We will verify your identity before acting on a request and will not treat you differently for making one. We do not sell personal information, and we do not share it for cross-context behavioral advertising.

Shine the Light. We do not disclose personal information to third parties for their own direct marketing purposes.

To make a request, call (747) 777-7047, email [email protected], or write to the address in Section 13. An authorized agent may make a request on your behalf with written permission.

10. Third-party sites and embedded tools

The Site links to and embeds tools from other companies, including the Tebra scheduling tool on our Book page, Google Maps, a Google reviews widget, and listings such as Psychology Today. Those companies have their own privacy policies, and this policy does not cover them. The scheduling tool is operated by Tebra under a business associate agreement with us; the information you enter there is part of your health record.

11. Telehealth and online booking

Video visits take place on a separate secure platform under a telehealth consent you sign before your first video visit. The Site does not host video visits and does not collect clinical information from them. A request made through the scheduling tool is a request; your appointment is confirmed when our office confirms it.

12. Changes to this policy

We may update this policy from time to time. When we do, we will post the new version on the Site with a new “Last updated” date. If the change is material, we will post a notice on the Site for at least 30 days.
