As a client of LA Integrative Nursing Inc DBA LA Integrative Psychiatry, you have the following rights under California law. These rights are established to ensure you receive the highest standard of care in a safe, respectful, and dignified environment.
1. Right to Quality Care: You have the right to receive safe, competent, and high-quality care from qualified and trained professionals. Your care should meet all applicable state and federal standards for safety, competence, and ethics.
2. Right to Dignity and Respect: You have the right to be treated with respect, dignity, and consideration at all times. Discrimination based on race, color, national origin, religion, gender, sexual orientation, or disability is strictly prohibited.
3. Right to Privacy: You have the right to privacy concerning your care, treatment, and personal information. Your medical and personal records are confidential and will only be shared with those directly involved in your care, unless you provide specific consent for additional disclosure.
4. Right to Participation in Care Decisions: You have the right to be fully informed about your condition, treatment options, and the risks and benefits of those treatments. You have the right to participate actively in decisions about your care plan and treatment.
5. Right to Informed Consent: You have the right to be informed about your treatment in a way that you can understand, including all risks, benefits, and alternatives. You must give informed consent before any treatment is provided, and you have the right to refuse treatment.
6. Right to Timely and Appropriate Treatment: You have the right to timely and appropriate treatment. This includes having your needs addressed as soon as possible, within the legal staffing ratios and facility capabilities, as outlined by California law (e.g., SB 349 for dialysis staffing).
7. Right to Safe and Clean Conditions: You have the right to receive care in an environment that is clean, safe, and properly maintained, with all necessary health and safety protocols followed.
8. Right to Voice Concerns or Complaints: You have the right to express concerns or file complaints regarding your care without fear of retaliation. The facility is required to provide you with a clear process for filing complaints, and you have the right to have your complaints addressed promptly and appropriately.
9. Right to Access Records: You have the right to access and obtain copies of your medical records and any other documents related to your care upon request. You can review your records during regular business hours or request copies for your personal use.
10. Right to Freedom from Abuse and Neglect: You have the right to be free from abuse, neglect, harassment, and mistreatment. LA Integrative Nursing Inc DBA LA Integrative Psychiatry has a responsibility to protect you from harm and report any suspected abuse or neglect immediately.
11. Right to Continuity of Care: You have the right to expect continuity of care, meaning your treatment plan will be followed, and efforts will be made to minimize disruptions. This includes transitions between different levels of care or caregivers.
12. Right to Refuse Services: You have the right to refuse any services or treatments offered by the facility, to the extent permitted by law, without being coerced, discriminated against, or penalized.
13. Right to Be Fully Informed of Your Rights: You have the right to be fully informed of all your rights upon admission to the facility. This information must be provided in a manner that is understandable to you, and you can request clarification or further explanation at any time.
Complaints and Grievances Process:
If you feel that any of your rights have been violated or that you are not receiving adequate care, you may file a complaint with LA Integrative Nursing Inc DBA LA Integrative Psychiatry or with the California Department of Public Health. You may also request an ombudsperson to assist you in resolving concerns.
